There has recently been a reported issue with the online triage form. If a patient was signed posted to A&E and completed an override of the system and submitted there form this might not have come to the practice. If you have submitted a form and not heard from us please call the surgery on 020 8336 6565
Please contact the surgery to inform the practice staff if you have any disability, impairment or sensory loss and require any assistance to book an appointment or make an admin request. Our booking system can be used by all patients aged 16 and over.

When booking an appointment please only discuss 1 problem per questionnaire.
Test results can be requested via the admin section or by calling our reception team.
